Tick Removal Permission Form

As we enter the warmer months, students will spend more time outside. At times, students may encounter ticks. Upon discovery of a tick bite, students are brought directly to the school nurse for care. Per the New York State Education Department, parents/guardians must provide permission for tick removal by the school nurse. This document provides information about school attendance.

Removing ticks within 24 hours of the time they attach to the skin reduces the chance of getting Lyme disease. If a tick is found at school, you will be notified by the school nurse. If the nurse can safely remove the tick, you will be provided with information about the removal and follow-up care. You may or may not need to pick up your student. 

Your permission is required for the nurse to remove a tick from your child at school. If you do not provide permission allowing the nurse to remove a tick from your child, you will be notified to pick your child up from school for tick removal.
