Millikan Summer League runs four weeks from July 22-Aug 16 having two sessions per week. Practice will be Tuesday and games will be Thursday. If we have a large interest, we will open another game night. Sessions will run from 6PM-8PM

We will accept team or individual registration. Donation fee of $50 for each player is necessary for referees, facility and insurance.

Everyone will need to have a US Lacrosse membership for insurance. You get many other benefits for joining. Please visit US Lacrosse at http://USLacrosse.org for details.

We will need your USLacrosse number so that you can finish this form. Please get that done before you start this form.
We will also need a COPY of your current physical form which will be checked on the first day (July 23).
