We, the parents or guardians of the above mentioned child, hereby grant permission for her to participate in the Lady Bulldog Basketball Camp and acknowledge the fact that she is physically able to participate in camp activities. We hereby release the camp and its employees and the school district from all claims from injury, illness, damages, or losses which may be sustained while attending camp. In making this foregoing statement, I hereby assume the risk of such program and do so knowingly and willingly with respect to my child.
