Teen Discipleship Team Form 2021
Thank you for your interest in being part of the St. Francis Life Teen, Teen Discipleship Team!

The Teen Discipleship Team is a select group of teens who are chosen to assist in leading, planning and executing St. Francis Life Teen events throughout the school year. The responsibilities of the Teen Discipleship Team include consistent attendance at Life Teen gatherings, a strong desire to assist others in growing closer to Christ and the ability to provide a welcoming and inviting atmosphere to those who are seeking to deepen their faith.

If circumstances change throughout the year and TDT members are unable to complete their responsibilities with the commitment and dedication required, both St. Francis Life Teen and the TDT member will have the opportunity to reevaluate how they are being called to serve in this ministry.

We ask that you fully discern your involvement in this ministry prior to completing these questions. While responding, know that all answers will be held in confidence and are strictly provided to assist the Core Members in selecting those teens that will best be able to serve St. Francis Life Teen in this capacity.
The Teen Discipleship Team will meet once a month, at a time to be determined by the members and Youth Minister.  The Teen Discipleship Team retreat will be Sunday August 15th.

Eligibility: Please initial on each line, showing that you meet each eligibility requirement. I meet the following requirements: Am an active, practicing, Confirmed Catholic (which includes going to Mass every week)
Eligibility: Attend Mass every weekend, preferably 6:00pm Mass
Eligibility: Participate in St. Francis Life Teen events, especially the weekly Life Nights during the school year
Eligibility: Attend monthly TDT meetings faithfully
Time Commitment: Please initial on each line, showing that you are willing to make each commitment. A member of the TAB will be expected to: Commit to daily prayer
Time Commitment: Love God, want to deepen my relationship with Christ & help lead others to know Christ more fully
Time Commitment: Will be a role model for my peers by making good moral choices, including abstaining from drugs, alcohol, sex, put-downs and gossip
Time Commitment: Schedule and attend additional meetings as needed to plan events

I understand this is a commitment to be on the Teen Discipleship Team, not only of my time, but I will also be called to be a disciple of Christ and to help lead other teens closer to Christ.
