YOGA LIABILITY WAIVER

I hereby agree to the following:
1. I am participating in classes, camping and/or services during which I will participate and/or receive information and instruction about camping yoga and wellness. I recognize that yoga and camping requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any camping, physical fitness program, including yoga. I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities.
3. In consideration of being permitted to participate in the yoga classes and camping trips, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of participating in the program.
4. In further consideration of being permitted to participate in the camping and yoga classes, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, the owner, or the leaseholder of the building for injuries or damages that I may sustain as a result of participating in classes or workshops.
5. That if I participate in other classes or events with Sacred Space Egyptian Yoga,  that I will also assume full responsibility for any injuries that may result from my participation, with the same considerations that this waiver stipulates for yoga (items 1-4 above).
I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.



Liability Disclaimer & Notices: please read carefully

I individually _________________________and as parent and/or guardian of any/the minor child____________________  hereby acknowledge the following notices and grant to Sacred Space Egyptian Yoga LLC  the following release from liability:

A. I acknowledge and fully understand that I, or my child, will be engaging in physical activities that may involve some risk of injury. I acknowledge and have been advised that it is my responsibility to consult with my or my child’s physician with respect to any past or present injury, illness, health problem or any other condition or medication that may affect my or my child’s participation. I assume the foregoing risks and accept full personal responsibility for any personal injuries sustained by my child which might incur as a result or participating in this program and discharge and hold harmless Sacred Space Egyptian Yoga LLC  its owners, directors, members, employees and the hosting facility from any claim, cause of action or liability for damages arising from any personal injury to my child or other persons or property caused by myself or my child’s participation.

B. Please note I use aromatherapy and pure essential oils in some of my yoga classes. If you are allergic to essential oils, advise me before each class. Please inform us of any allergies you may have as well.

C. I agree / disagree to give Sacred Space Egyptian Yoga LLC  permission to use photographs of my child or myself for any Sacred Space Egyptian Yoga LLC promotional materials. I understand that my child will not be identified by name, nor will any compensation be extended for such use.

D. Please convey the following information to your child: Asana (yoga posture) means posture easily held. If it’s too hard or if it hurts, you can stop! You may rest at any time during the class. It is important in yoga that you listen to your body, and respect its limits on any given day.
