QUALIFYING CRITERIA AND INSTRUCTIONS
QUALIFYING CRITERIA:

HORSE - 1. Nominee must have been outstanding over a period of years as a sire, dam, or competitor.  2. Nominee must have been owned by a resident of the State of Oregon.  3. Nominee must have been registered as an American Quarter Horse.  4. Nominee must be deceased.

HUMAN - 1. Nominee must be or have been, over a period of years, significant in the growth of the Oregon Quarter Horse industry; or have been outstanding in his/her area of activity involving the American Quarter Horse in Oregon.  2. Nominee honors or awards at the AQHA level may be considered in part but will not be the only criteria for induction.  3. Nominee may be living or deceased.


FORM INSTRUCTIONS:

Section 2:
Required Information.

Sections 3 and 4:
Fill these sections out if nominating a HORSE: Include biographical information, such as date of birth/death, show records, achievements, honors, awards, prominent offspring/achievements, and articles from the AQHA Journal, AQHA Racing Journal, etc., and endorsement letters.  Include any other information which you deem significant.The more you put into your nominating effort, the better the chance for your nominee. You may use as many pages as necessary. Submitted information and documents will not be returned to you.

Sections 5 and 6:
Fill these sections out if nominating a HUMAN:  Include biographical information, such as equine association activities, offices held, honors, prominent horses owned and their achievements, civic activities/honors, military service/honors, and endorsement letters.

Section 7:
Submit photos to be attached to your nomination form.


IMPORTANT:

The more you put into your nominating effort, the better the chance for your nominee.
