Parents are requested to complete the relevant sections of this Form on admission of their child to our School. The information collected:

1. Ensures that School Staff are aware of any learning and language needs your child may have.
2. Ensures that the cultural linguistic and religious needs of all pupils are included in the ethos of the School.
3. Is required by the Department for Education (DfE) in order to monitor Standards of Achievement.
