WAIVER AND RELEASE OF LIABILITY

AND ASSUMPTION OF RISK ACKNOWLEDGEMENT

The undersigned A Grateful Mind International Volunteer, (PRINTED NAME BELOW)  agrees that they are responsible for their personal vehicle and possessions while volunteering; and hereby releases any and all liability for injuries, collisions, theft or property damage and waives the right to expect or request compensation from A Grateful Mind International (AGM) and/or any of its associates, officers, partners, clients, other volunteers or employees that arises out of, or is connected with, or in any manner relates to, their personal possessions and/or driving their personal automobile to or from any Volunteer site locations. 

The undersigned agrees to be in possession of a current state driver’s license, have the appropriate registration and proof of automobile insurance as required by all applicable laws.

Undersigned further acknowledges that AGM does not authorize any of the aforementioned person(s) to alter, imply or infer changes to the terms of this waiver at any time hence or in the future; nor does AGM accept or assume responsibility for any maintenance involved for the undersigned's personal automobile use while working as a volunteer currently or at any future date should the undersign discontinue volunteering services with A Grateful Mind.
