Please complete the following information.  Most of these sections must be updated every year.  Those sections are identified with a note in the description and are required in this form.  Some sections are only for students transferring in or K students.  Other sections are for particular grades.  The last section contains several linked documents that may require you to print the form and submit a copy to the office.  

For new students (Kindergarten or students transferring in) a copy of the BIRTH CERTIFICATE and IMMUNIZATION RECORD MUST BE SUBMITTED PRIOR TO THE STUDENT BEING ALLOWED TO START.  

Registration is NOT complete until the Student Records Release Form and Proof of Residency Form are filled out and submitted to the office.  Links to print these documents can be found at the end of this form.
