INJURY WAIVER AND GENERAL RELEASE FORM 

I give my permission for my son/daughter to participate in the Jus’Dance Academy @ Just the Place, Inc. programs and performances. I understand that although the students will be supervised by staff, I do assume the risk in my student’s participation in this program. By signing your name below as a participant, and/or parent / legal guardian of a student at Just the Place, Inc. I acknowledge that I will not seek to have Just the Place, Inc., officers, directors, employees, agents, consultants, independent contractors and affiliates held liable in the event that any accident, injury, loss of property or any other circumstance or incident occurs during or as a result of my son’s/daughter’s participation in this program. This release of liability includes accident, injury, loss, or damages to the student, as well as, to other individuals or property which may result from the student’s participation in this program. I hereby release and agree to hold Just the Place, Inc. its officials, agents and employees, from any claims arising out of my son’s/daughter’s participation in this program. I have read and understand and accept all of the statements above and accept full responsibility as described.

 I understand that my child’s participation in the activity requires following ALL studio rules as well as exhibiting acceptable, appropriate behavior at all times. I acknowledge that I have reviewed with my child the rules and requirements established for this activity; all policies and procedures; rules of conduct set forth in the Student Code of Conduct, and that they understand the need to abide by them in order to participate.

 Students should never be left unattended at any time. Please do not drop your child off any earlier than 10 minutes before class and all students must be picked up promptly after class.

 You acknowledge that you have read and fully understand the injury waiver and General Release Form. This agreement shall be binding on you, your spouse, your children, legal representatives, heirs, successors and assigns.

STUDENT CODE OF CONDUCT

ATTENDANCE is crucial. Each missed class is missed material, information and necessary practice. *
ABSENCES - We understand that everyone gets sick and has important occasions that will cause students to miss class once in a while. We ask that you give us as much notice as possible.  *
ARRIVE early for classes. Please arrive dressed for class. The dressing area is small. Make sure you use the BATHROOM BEFORE CLASS! *
CLASS ATTIRE - There is a dress code (see class attire form). We ask students to wear clothing that allows the instructors to see correct alignment and placement. 

* JTP T-Shirt, Ballet Dress, Jazz Pants, etc...
* No dangling or large jewelry.
* Hair should be up and pulled away from your face.
* Appropriate shoes should be worn for specific classes.

INCLEMENT WEATHER POLICY - We notify you in advance if we close due to inclement weather and payments will be adjusted for any cancelled classes.  *
GROUPME APP - Please download the "GroupMe" App. It is the best way for to communicate as a studio. *
ELECTRONICS - Cell phone use is not permitted during class. Phones are to remain in backpacks or with belongings. If students need to make a call, a staff member will supervise use and require phone to be put away before the student rejoins group. If a student is found using electronic devices during program time, the device will be removed and kept by senior staff until the student is picked up at the end of their session. *

CLASSES AND FEES 

* Annual Registration Fee: $30 Annual (calendar year) Registration Fee is non-refundable.

* Dance Class Tuition Fee: $10 - $15 per class; 45 minutes - 1 hour of class each week.

* Class tuition is due on the 1st of every month. After a 5-day grace period, a Late Fee of $5 will be charged.

•  We accept Cash, Check and Credit Cards, Debit Cards, PayPal.
•  Tuition payments are non-refundable, and we do not offer reimbursement or makeup classes for classes missed. 
•  Students will not be allowed to take class if their account is delinquent for more than 30 days.

HIGHLIGHTS & LOOKING AHEAD

* JR. AND SR. ELITE TEAM AVAILABLE - Elite team members will be chosen by studio staff and instructors via class participation and an audition. Elite Teams members will pay an additional fee for their participation as well as an additional Costume Fee. Parents of Elite Team Students must agree to the rehearsal and performance schedule. Placement in The Elite Dance Company is based on the following criteria: technique, potential, commitment level, enthusiasm, attitude and behavior in class, attendance and stage presence. We place students together in groups that we feel will work best together in rehearsals and performances. Company members are selected by instructors and will be invited into the appropriate Company class.

 * RESPECTED STAFF - All of our staff members and guest teachers are experienced instructors, dancers, choreographers and performers.

 * NON-PROFIT STATUS - This is perhaps our number one advantage. Our Dance Program is and will continue to be a non-profit program. We will look for grants, ask parents and the community for support and try to establish as many programs that will support our Mission and Goal. We will conduct ONE fundraiser per semester to help support the program. Your support enables us to maintain the lowest tuition in the community.

 * RECITAL - Our Annual Dance Showcase usually takes place the first weekend in June @ The Peekskill Middle School. Tickets cost approximately $40 per person. A ticket is required to watch each recital. This payment covers the cost of auditorium rentals, printing fees, staff to run the show and vendors for set-up, tear-down and lighting.

 *COSTUME FEES - Costumes costs on average are $65 to $90 per class. Costume deposits (50% of the total costume cost) are due in January. The balance is due upon receipt of the costume.
