2021-22 Private Lesson Assistance Form
The Wakefield Music Boosters Association, Inc. is an organization whose purpose is to support the music & performing arts programs within the Wakefield Public Schools. As part of that support the organization provides assistance to students participating in the instrumental and vocal private lesson programs that may otherwise not be able to afford private lessons.
 
The following are the guidelines for lesson assistance:

-The program is needs-based.
-The student must attend the Wakefield Public Schools in grades 5-12.
-It is recommended the recipient & parent(s) volunteer at 2 Music Booster events.
-Assistance provides for 2 lessons per month.
-Students must attend scheduled lessons and come prepared to learn.
-Private lesson teachers are required to provide documentation monthly to the Director of Visual and Performing Arts in order to be reimbursed for lesson time.
-Up to 2 members per family may participate in the program at any one time.
-Acceptance into the program is on a first-come, first-served basis and you will be notified by October 1 if you have been selected for the program.
