Parents - please fill this form out as thoroughly and accurately as possible. As our Faith Formation Program is for the WHOLE family, please include ALL of your children on this registration, not only those preparing for sacraments this year. However, please also note that the program for 7th & 8th grade / Confirmation preparation is in many ways a separate program and will require a separate registration (stay tuned for more information). If there is information you need to share with us that does not fit into any of the particular questions or categories, please add it in the notes at the end of the form. The structure of our parish's Family Faith Formation program involves small groups of families who will be grouped together for conversation, activities and support/encouragement. Please list the name(s) of ONE or TWO families that you would prefer to be in your small group. PLEASE NOTE: groups will be formed based on multiple factors, and we do not guarantee that all families who desire to be grouped together will be able to be, but we will do our best! *

Family Faith Formation is all about growth and encounter - together, as a family! Our parish wants to help your family to grow in faith this year, in the way that the Lord is specifically calling your family to grow! Please pray about what the Lord is calling you to, and then explain below one or two specific ways that you hope your family will grow in faith this coming year. For example, if your family does not attend Mass each Sunday, your goal may be to attend Mass each Sunday this year. Or, if your family does not pray together or have conversations about faith regularly at home, your goal may be to do these more regularly and to set a particular time for these each day/week this year. Or, your family may wish to make time weekly or monthly for Eucharistic Adoration or Confession as a family. Or, your goal may simply be to make attending each Family Faith Formation session or doing your Family of Faith lessons at home more of a priority this year. Whatever your goal is, please explain it below so that our community can help you achieve it!
