Please complete this application only if you are able to commit to all of the session dates and three Learning Team meetings (after sessions A, B, and C).
Applications are due by
5pm on Wednesday, April 10, 2024. Leaders will either be accepted or added to a wait list for a future ALP cohorts. All applicants (and/or a support contact) will receive an email by
Friday, April 19, 2024 letting them know if they have been selected.
This information is kept confidential. We will use the data for overall program statistics that are shared with our funding sources and collaborative partners. We will not share individual information that could identify a specific person.
