PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

CHALFONT ST GILES JUNIOR SCHOOL – YEAR 4 

Year 4  Frisbee PE Festival at Amersham School, Stanley Hill, Amersham, HP7 9HH. 

All of the children in Year 4 will be participating in a Frisbee Festival at Amersham School on Tuesday 12th March between 10:15am -12:30pm (event time is from 10:45am to 12:00pm). The children will be travelling to and from the festival by coach. 

Children will leave the PE Festival at around 12pm and will be able to have normal lunch.

Children should come into school wearing their school PE kit (green T-shirt, black shorts or black jogging bottoms and green hoodie) and trainers.

Children will also need a named water bottle to take with them.

We will need parents to help on this trip. If you would like to help and have a current school DBS please contact the school office.

By completing this form I am giving consent for my child to attend the Year 4 Frisbee Festival at Amersham School on Tuesday 12th March. I confirm that I have read the relevant information above. Consent forms to be completed and returned by Friday 1st March.

I agree to his / her participation in the activities described and I acknowledge the need for and expect him / her to behave responsibly. I can confirm that the school has current contact and medical details for my child.
