This survey is for students who have received F-1 transfer-pending initial status or change of level SEVIS I-20s from Pratt Institute for Fall 2020.

Please complete the following so that we can know your plans and what to do with your SEVIS record. We ask that you submit the form as soon as possible (by August 9) so that we can be aware of your intention at this time.

I am not planning to take classes in Fall at Pratt. I will withdraw or defer my admission if I have not already.

Is there anything else you would like OIA to know about your situation pertaining to the above question? For example, if your F-1 Visa has expired and you are applying for a new one, do you have a visa/consulate appointment date? If so, when and where is it scheduled?
