(ONE FORM PER PET)      ** Pets must be current on all requirements in order to be evaluated for daycare & MUST be spayed/neutered. Please fill out the following form to the best of your ability. If you have any questions please let our staff know. Please complete this form if you are interested in our daycare program. Daycare is Monday-Saturday, and consists of two separate sessions. AM session is 9:00am-11:30am, and the PM session is 2:00pm-4:30pm. Each guest must pass an evaluation PRIOR to be accepted into our daycare. **Not every dog qualifies for daycare.

Bartels Busack Pet Hospital Resort & Spa requires daycare guests to be CURRENT on the following to be eligible for daycare: Rabies, Distemper, Leptospirosis, Bordetella, Canine Influenza, NEGATIVE Fecal within the last 6 months, Yearly Negative Heartworm Test, and Flea & Tick Medications. If a guest is not current on one or more of these they will not be allowed into the daycare program until updated. Records must be dropped off prior to evaluation. We also require pets to be spayed/neutered after 9 months of age, no exceptions.   *

  • Every dog will react differently and animals in nature are unpredictable. There is a certain level of risk associated with day care which may include but not limited to injuries to people, property, and animals resulting from dog fights, bites, rough play, and transmission of disease.

  • Dogs not accustomed to being away from home may experience separation anxiety and may need an adjustment period before being allowed to socialize with other dogs in daycare.

  • Your pet must be okay with staff handling them in order to be in the daycare program, this is for their safety, the staff's safety and the safety of the other guests.

  • Dogs may experience sore muscles, joints and fatigue after daycare due to high levels of activity.

  • Each pet will be required to be evaluated for daycare on their first day or if the pet has not been in daycare for over 90 days.

  • Dogs that have not been socialized may not know how to behave with other animals, which may make them at higher risk of starting dog fights, biting, fear related aggression, object guarding, behavior problems, signs of stress/fear and acting out. Any guest that shows this type of behavior during their evaluation or time in daycare will be dismissed immediately.

  • Any behavior deemed dangerous to the pet, staff or other daycare members will not be tolerated.

  •  Due to weather conditions or your pets level of activity, your dog may get dirty, matted, or wet while here for daycare. Grooming/Baths can be scheduled at an additional charge.

  •  There is a risk to puppies who are not fully vaccines who are admitted into daycare.  

  • When leaving daycare he/she may be very thirsty or hungry. Excessive food or water can cause an upset stomach.

  •  You agreed that all information in this form is true to your best knowledge and your pet is in good health and not exposed to any contagious diseases in the last 30 days.

  • To the best of your knowledge your pet has not shown any aggressive or threatening behavior towards people or other dogs.

  • The Resort staff at Bartels Busack Pet Hospital Resort & Spa are not certified veterinarians or trainers and are not liable for the treatment or diagnosis of any injuries that incurred while my dog was at daycare.

  • If my dog is involved in an incident, I will have my dog checked or treated by a licensed veterinarian at my cost. 

  • We may take photographs of pets for social media or marketing purposes. You may opt out of this below.

  • Finally, you agree to pay for all services at pickup associated with daycare, boarding, hospital services, grooming and any add ons.
