Please only use this Form to submit a Health Report if you are a UK Finnish Lapphund breeder or owner.

You can use this Form to enter a Health Report for ONE Finnish Lapphund (born any time from 1995).  Please submit a new Form for each additional Finnish Lapphund.

By using this Form you agree to the Breed Health Coordinator using the data you supply to inform the Finnish Lapphund Health Plans and to provide reports on the health status of UK Finnish Lapphunds.

Your personal details and those of your Finnish Lapphund(s) will not be shared publicly and will only be used, in confidence, by the Breed Health Coordinator.  (Forms can be submitted anonymously if preferred).

The results of the survey will be published, as part of the Breed Health and Conservation Plans and on the Finnish Lapphund Health website  - but the results will be presented so individual dogs cannot be identified.
