如有任何問題都煩請填寫此表單，以下特別注意：

❗中英文姓名（只能修改錯字）
❗分齡組別 (性別、分齡是否正確)
❗身分證字號（只能修正錯誤）
❗選手務必至 Challenge Asia Pacific APP 將選手本人手機號碼進行綁定，即可同步查看選手資料。
提醒您：APP綁定之手機號碼和報名填寫電話必須相符，否則將影響報到手續與比賽權益。

如需修改選手資料請於『 3/12 11:59pm 前』填寫表單，逾期不受理。
大會將於3-5個工作天使用信件方式進行回覆,煩請選手們耐心等候！

The list of athletes is now available! Athletes are kindly reminded to carefully check their information, and not to forget their basic rights and obligations as athletes.

If there are any issues, please fill out this form. Please pay special attention to the following:

Chinese and English name (can only be corrected for spelling mistakes)
Age group (gender, age group correctness)
ID number (can only be corrected for mistakes)

Athletes must bind their personal mobile phone number in the Challenge Asia Pacific APP in order to view your information synchronously.

Please note that the bound mobile phone number in the APP must match the phone number filled out during registration. Otherwise, it will affect the registration procedure and the competition benefits.

If you need to modify athlete information, please fill out the form before "11:59pm on 3/12". Late requests will not be accepted.

The organizing committee will reply by email within 3-5 business days. Athletes are kindly requested to be patient and wait!
