Fill out this form if you would like to be a part of the SFT Family Directory. It is 100% optional and only information given on this form will be included in the directory. We will not collect information from any other source. If you do not fill out this form your information will not be included in the directory. The information from this form will only be used for the 2023-24 SFT Family Directory. If we create a directory in future years a new opt-in form will be used. This directory will be distributed to current SFT families only. The directory will be distributed electronically.

This will be distributed over multiple platforms, if you receive from multiple sources there is no need to fill out each time.
