This short questionnaire is to gauge interest in those who would like to receive the COVID-19 Vaccination.  Individuals that are 12 years of age and older are eligible to receive it.  King of Kings has the opportunity to host a vaccination clinic but must have a specific number of people that are interested.  If enough people are interested, the clinic will happen in October.  Please share this with your networks
