SADOC Small Claims Form
This form can be used to report a loss of goods up to $10,000 in value.

Claims greater than $10,000 must be made via the main Internal Affairs reporting method. If your claim is greater than $10,000 in value please use the form found here: https://gov.eclipse-rp.net/viewtopic.php?f=1194&t=29139

Blatant and/or repeated misuse of this form will result in a suspension/ban from further submissions.

Acknowledgment
This report will be reviewed by the SADOC Internal Affairs department. Claims must be submitted no later than 8 days ((48 hours)) after release from Bolingbrooke Penitentiary.  Any claim made after this 8-day ((48-hour)) period will be instantly denied. A member of the SADOC IA team will reach out to you via email ((forum pm on the government website)) within 12 days ((72 hours)) of submission to let you know the result of your claim. Claims between $5,000 and $10,000 in value receive a higher level of scrutiny and therefore may require the filing of a full IA complaint form if deemed necessary. If your claim is accepted by the Internal Affairs department, you will be provided instructions via email ((forum pm)) on how to claim any reparations afforded to you.
