Thank you for taking the time to complete the Lakeland Hockey Association's Coaching Evaluations

The survey should only take a few minutes, but your feedback and honesty helps ensure that Lakeland is continually improving and providing the best coaching options for our players and families moving forward.

It also helps to give our coaches a chance to learn and improve from the feedback provided.

The results will be shared anonymously with the coaches after the survey period is complete and reviewed by the Hockey Director.

Your name and written comments will only be seen by the LHA Hockey Director.
