Please complete this form to start the student registration process at FCRSD.
Once we receive your registration information, you will be contacted by the Elementary Office to schedule an in-person appointment.
Parents/guardians must bring the following REQUIRED documentation to FCRES to complete the registration process: student's birth certificate, proof of residency in district (utility bill, rental agreement, sales agreement), current immunization record, and for Pre-K ONLY last year's Federal Income Tax adjusted gross income.
During the scheduled appointment, parents/guardians will finalize registration while students meet with teachers and clinical specialists to assess current levels and any individualized needs your child may have.
It is important to remember that enrollment in The Forest City Regional School District cannot be completed without the required documentation.
Also, please note that any student who was enrolled in Pre-K at FCRES does NOT need to be registered for Kindergarten.
