This form may be used to register swimmers for the next Collaroy Swimming Club race meeting.

Please do not register any swimmer unless you are certain they will be swimming. If a registered swimmer cannot swim, you must cancel the registration. Swimmers who register and do not swim on the day may lose online registration privileges.

Strokes are shown in the order in which they will be swum.

You may enter multiple registrations for a swimmer - only the last one will be processed.
