Upon completing and submitting this registration form your child will be registered for preschool. We will hold his/her spot in the class for two weeks.  Your child's spot in preschool will be permanent after we receive:
                1.) Child Enrollment and Health Information Form
                2.) Family and Personal History Form
                3.) a copy of his/her birth certificate,
                4.) registration fee of $60.00 and
                5.) first month's tuition.
(Your child's medical form is not due until the first day of school.)
You may mail them to the preschool or drop them in the locked mailbox located to the right of the door at Shoregate United Methodist Church.
