I, the above mentioned parent and my child, the above mentioned participant, desire to participate in the course specified above. I understand the above mentioned program offered through The Deen Team will take place in a wilderness/outdoor environment and may include but is not limited to, the following potentially hazardous activities: Paintball and similar activities. These potentially hazardous activities can cause personal injury, property damage, death or illness. I understand that participation in this course is voluntary. In recognition of the potentially hazardous nature of the elective course, I, and my child, my heirs and assigns, hereby release The Deen Team, the Venue and its employees/volunteers from all claims and liability from negligence arising from participation in the course. I further agree to hold harmless and indemnify The Deen Team and its affiliates, staff and agents for all defense costs, including attorneys, and any other costs resulting in connection with my participation. I understand that this release relates to all claims and liability during or after the trip resulting from a pre-existing medical condition. I have read and completed fully the medical form provided by The Deen Team (on reverse of this page) and accept full responsibility for omissions or errors on the medical information form. I also give permission for the organizers to act at their discretion - should an emergency situation occur in terms of ensuring the best safety or care for my child. I also understand that this release relates to all claims and liability resulting from unforeseen or intemperate weather. I have read the clothing and equipment list (Kit List) provided by The Deen Team and accept full responsibility for inadequate clothing or equipment provided by me or which I fail to provide. I have read this entire release of claims and fully understand it. I have satisfied my questions and concerns regarding the above mentioned course by talking with representatives of The Deen Team. * *
