We've got plenty of educational activities planned for this summer!

Activities will consist of Hands-On: Discovery, Arts & Culture and more.

REGISTRATION FEE OF $35 TO RESERVE SPOT AND MATERIAL. (NON-REFUNDABLE)  

Payment can be paid by (debit cards, visa, Mastercard (an invoice will be sent to pay by debit or credit card), Apple Pay, Cashapp($Smiley2007), Venmo (CJ_Resoursources), Zelle (Chante Myles)

The C&J Resources Summer Day Camp Program runs from 
July 8- August  2, 2024
Monday-Friday, 7:30 a.m.-5:30 p.m.

Cost:
Summer Learning Camp 
$60 a day/$100 Sibling a day
$180 a week/$350 a week siblings
$25 for additional third child plus

OR

Tutoring 

Tutoring starts at $45 an hour (depending upon grade) 
 
PROVIDE YOUR CHILD(REN) LUNCH and AT LEAST 2 SNACKS FOR EACH DAY!! 

PLEASE NOTE: Payment is due at the beginning of each week and there will be a late fee for any pickups after 5:30PM and $1 every minute when you are late.

I hereby give my permission for (name of participant/child/ward) to participate in this program/activity conducted by the C&J Resources. I assume all risks and hazards incident to such activities and transportation to and from the same. I release,
discharge, and acquit CJR and all of its agents, servants, employees, staff and personnel from and with respect to all claims, causes of action and rights of
recovery which I have, or might have at any time in the future as a result to any property damage or bodily injury suffered by said child/ward during the course of
any such activities. Additionally, I agree to indemnify and hold harmless CJRnd the employees from and against any and all claims, suits, damages, judgments,
attorney fees and expenses of every kind on account of property or bodily injury, including death, suffered or experienced by me or my said child/ward occurring
during, or in any way resulting from any of said activities, whether or not caused by negligent acts (except as may be occasioned by gross or wanton employees)
or omission of any sort by CJR employees. I authorize CJR and employees to render any medical care and treatment to my said child/ward deemed necessary
with respect to any illness or injury occurring during any CJR activities. I fully understand that CJR has NO ACCIDENT or MEDICAL PAYMENT INSURANCE
COVERAGE for the participant/child/ward and I agree to pay all medical costs incurred if treatment is obtained. Furthermore, I will take caution when sending
valuable/sentimental items with my child/ward to any activities, and understand that C&J RESOURCES accepts NO RESPONSIBILITY for lost, stolen, or misplaced items. CJR
does not discriminate based on disability in the admission/enrollment or access to our programs. However, if an individual poses a threat to the health or safety of
others or would cause us to fundamentally alter our program,, we would have to determine whether reasonable accommodations could be made to integrate the
individual into our program. I give permission for CJR to take photographs and/or videos of my child during activities for publicity use.
