Foster to Adopt Application Friends of the Animals
We are now requiring an application be filled out BEFORE making any appointments with FOA to see animals so we can better serve our pets. 

Please fill out this form if you are interested in adopting a pet from FOA. Fill out each question COMPLETELY and accurately.  Please do not fill out in your name if you are not the one wanting to adopt we only accept applications from the person who is adopting the pet.  After your application is received and has been reviewed we FOA will contact you if we feel you are a match for the pet or if the pet is adopted we will pull the pet from our website or site you saw it on. 

IMPORTANT!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!  All applications not filled out completely as directions stated on the application will be bypassed and no notifications will be sent to save time for others that do follow instructions as our time is extremely limited and we need to serve the animals in the best way we can and need this process on your part to run smoothly.  Thank you!

Please give the name of the animal(s) you are interested in.  If interested in more then one please put in the order your interested.  Sometimes your first choice may get adopted out before your application can be verified and we will then move to you other choices.

Please read the following paragraph regarding the foster to adopt process.

FOA requires all dogs above age 2 and special needs cats undergo a 2 week foster before allowing anyone to adopt. There are a few exceptions to this rule and not limited to if you live out of the state of Iowa, if the canine is below the age of 2 in certain circumstances and some puppies.  This period can be extended if we feel it is in the animal’s best interest. 

We require this to help you transition the animal into your home to make sure the fit is right for the animal and your family.  Our foster to adopt program can help you with this process to make the adjustments easier for all. Please note though the program isn't a trial period to see if you want any animal example:  not sure we are ready for a pet trial as our pets need life long commitments.

FOA has the best interest of all parties at heart and wants this experience to be pawsitive for everyone. We try our best to help as much as we can to make the process a good experience for all.  We are easy to work with and want you to know we only want what is best for our furbabies.
