2024 MOA Awards - Nomination Form
Each year, MOA recognizes those who contribute to the betterment of Maryland Optometry during our Annual Membership Meeting. Awards are provided for four categories, Young OD of the Year, OD of the Year, lifetime service (Mel Waxman) and public service (given to someone outside of the profession). A description of each award is provided by section below. Recipients can only receive each award once. A list of prior award recipients can be viewed on the MOA Website at https://www.marylandoptometry.org/page/PastAwards.

Members can nominate themself or another doctor for each award by providing the recipient information requested below prior to April 15, 2024. To nominate multiple candidates for one or more awards, a new form must be completed per nomination. The MOA Awards Nomination Committee will review the list of all qualified nominees for each award and make their determination for 2024 winners. Winners will be recognized and presented with their award during the MOA Annual Membership Meeting Luncheon at the Family Fun Summer CE Program at the Great Wolf Lodge in Perryville, MD on Saturday, May 25th.

Melvin Waxman Award
Presented to an optometrist in recognition of his/her dedicated and loyal service to the profession and to the Maryland Optometric Association

This award recognizes a deserving individual Doctor of Optometry for performance of outstanding services on behalf of the profession and to the visual welfare of the public
