This form should be used to communicate Intention to Withdraw for students who have PREVIOUSLY been signed up for CONTINUOUS ENROLLMENT.
This form can only be used for a ONE student. (If you indent to withdraw multiple students, please submit this form multiple times.)
Please be reminded that in accordance with the Continuous Enrollment Agreement, this form must be submitted before April 15 to avoid late withdrawal fees. Forms submitted after April 15 will be subject to the fees outlined in the Continuous Enrollment Agreement.
