Liability Release - I acknowledge that in consideration of the acceptance of this pass for participating in this activity, I agree to release & indemnify Steps to Stardom Performing Arts Academy & its instructor’s as follows - my child will participate in the activities at my own risk & responsibility.I understand that dance can be a dangerous & hazardous activity due the active nature of the sport & my child is participating at my own risk.I release, indemnify & hold harmless Steps to Stardom Performing Arts Academy & its instructor’s against all & any actions or claims which may be made by me or on behalf or by other parties for or in respect of arising out of injury, loss or damage caused to me or my property with the exception of any case where we are found negligent. I also agree that in the event that my child is injured on the premises in which they are training or my property is lost, stolen or damaged, I will bring no claim, legal or otherwise, Steps to Stardom Performing Arts Academy or its instructor’s. I am sending this agreement with the understanding that I have read & understood the above terms & conditions. *
