We at Peninsula Tennis Academy recognize our obligation to make our players and their parents aware of the risks and hazards associated with the sport of tennis. I fully understand, and agree to make my child(ren) aware, that tennis can be a dangerous sport, and that players may suffer injuries- possibly minor, serious or catastrophic in nature, with injuries including but not limited to dental injuries, medical injuries such as twisted ankles, wrist injuries, concussions, and even death. Peninsula Tennis Academy, its coaches, owners, other staff members, volunteers, or employees, volunteers, elected officials or other representatives of the cities of Burlingame, San Mateo, Millbrae, San Bruno, Foster City, or Hillsborough will not accept responsibility for injuries sustained by any student or spectator, before, during or after tennis activities. With this information in mind, and being fully aware of the risks and possibilities of injury or death involved, I consent to have my child(ren)/ myself participate in the programs offered by Peninsula Tennis Academy. I, my executors or other representatives, wave and release all rights and claims for damages that I or my child(ren) may have against Peninsula Tennis Academy and/ or its representatives, whether paid, elected or volunteer. I also affirm that I now have and will continue to provide proper hospitalization, health, accident, dental and vision insurance coverage which I consider adequate for my child(ren)'s protection and my own protection. I also understand that it is the parent's responsibility to warn the child(ren) about the dangers of tennis and injury. Parents should warn child(ren) to the degree that they feel is appropriate. Peninsula Tennis Academy will only warn the child(ren) through safety messages and our USTA recommended coaching style.
