We are providing this form for you, so you can help us send in storm reports, that way we can relay your reports to the correct National Weather Service Office. Any reports can be sent in to us, including damage reports, flood reports, snowfall amounts, etc.

Ideally, we recommend that you send your report to the National Weather Service for your area. If you are unsure of who your NWS office is, please contact us via Facebook or email us at ohweathernetwork@gmail.com. You will need to provide a town or county for us to do this. At the end of the form, we ask for your name and email to help with verifying storm reports and keeping track of those who abuse the storm report system, such as sending false storm reports in. This will help us follow up with your storm report if the NWS has any additional questions about the storm report.

Disclosure of personal information is completely voluntary and will be forwarded to the NWS only with your consent, which will be asked at the end of the form.
