Thank you for completing this form to request help finding an appointment for the COVID-19 vaccine. A volunteer who speaks Mandarin/Cantonese will reach out to you by phone shortly. Please have your phone available for the next 72 hours so that you are ready for the call. The volunteer will identify themselves as a WGIRLS volunteer and then ask you questions to complete the information required for you to schedule an appointment! COVID-19 Vaccines are available for free to everyone living in the United States. Our volunteers will never ask for insurance information, your social security number or information regarding citizenship.

IMPORTANT: Please DO NOT complete this form if you are in a position to help yourself, or someone you love, secure an appointment. This is a small group of volunteers who are seeking to identify and support those who don’t have anyone else to help them.  If you submit this form for someone who could help themselves, or whom you could help, you are taking resources away from those who TRULY need us.
