$45 per person (not including alcoholic or specialty drinks)
Capacity: 40 people

Dinner will be held in a private indoor room with a limited menu, including options for starters and entrees as well as various shared appetizers. Several vegetarian options are available.

Because space is limited, RSVPs will be collected on a first-come first-served basis.

Each attendee MUST RSVP individually AND either complete payment directly OR have payment completed on their behalf* to secure a spot at dinner.

For your spot to be reserved, please remit payment ($45 per person) to either PayPal (pierreazzam@gmail.com) or Venmo (@Pierre-Azzam) upon completion of this form. For confirmation, my telephone number is 617-768-7498 (you may be asked for the last 4 digits in order to send payment).

Pre-payment is refundable up to 48 hours in advance of the event (i.e., August 3 at 7:00pm CDT) or transferrable at any time, with request to pierreazzam@gmail.com or 617-768-7498.

*Please note that while mentors are permitted to remit payment on behalf of their students and trainees, they cannot ask for spaces to be held by proxy. Each RSVP will be considered based on the order in which it was received.
