2024 - 2025 TMOM Membership Renewal
This form is for TMOM member clubs to renew their dues for 2024 - 2025.  Please complete the following three steps:

1) Complete this form

2) Email your club member list & officer list to the TMOM Membership VP (membership@tmom.info).

    Please use Excel, a template for both lists is in the membership renewal section of the website
    at tmom.info/membership-renewal

3) Submit dues by one of two methods:
    
    PayPal  (You will be issued an invoice based on the information you provide in this form.)
    or
    Mail a check to Missy Khan, 18011 Stoney Glade Court, Houston, TX 77095
 
Renewal form, membership & officer lists and payment must all be received to complete your renewal.
