All information and 8 weeks of 2 strategies must be completed before the student can be referred to special education. Please see Amber K. or the SIT team if you need help finding strategies to meet student needs.
The form is set so that responses can be edited after submitting so you can go to the email receipt that is sent to you upon submission and update your form to collect data as you go OR here is a link to a "paper copy" of the form if you are more comfortable keeping that and then entering the data into this form. There are copies of these
intervention manuals throughout the elementary building and in Amber K's office if you need ideas.
