I certify that all players are of the correct age and acknowledge that I must carry proof of age and eligibility with me at all times. I agree to be ready to produce this proof if asked by the referees or tournament officials and/or staff. I understand that proof of age and eligibility consists of laminated player cards with pictures issued by either USYSA, US Club Soccer, or USSSA (virtual cards with pictures will also be accepted). If I do not have player cards, I agree to provide proof of registration in the form of a letter from my club registrar certifying that our player is indeed registered and eligible for the 2023/2024 seasonal year. The letter will be on club stationery and include team name, player name, player date of birth, and player ID number. This letter must include all players for which I do not have player cards. I acknowledge that failure to provide proof of age and eligibility will result in a forfeit.

I acknowledge that I must have medical release forms in my possession for all of my players in case of injury or emergency.

I acknowledge that all game cards will be turned in by the winning team, or home team in case of a tie, no later than 30 minutes after the conclusion of the game. I agree to check the game card at the conclusion of the game to verify correct scoring.

I acknowledge on behalf of our team that our spectators will fall in line with fair play and will support players, referees, and tournament staff accordingly as verbal abuse of anyone will not be tolerated. *
I acknowledge that all players must be listed on my roster in Got Soccer, including guest players, and that this will serve as my official roster. I understand that ALL PLAYERS MUST BE ADDED AND ROSTERS SUBMITTED BY TUESDAY, FEBRUARY 6, 2024, 5:00PM. I will make sure to print a game card by this date to verify that all players are listed. I acknowledge that NO WRITE-INS will be allowed and that write-ins will result in a forfeit.
