We, the undersigned, give our child permission to participate and do, by affixing our names below, hereby release West Orange Cove CISD and all other persons associated with the 2023 WOS Mighty Mustang Football Camp from all liabilities for bodily injury or damage either physical or mental, resulting directly or indirectly from any means or cause and affecting the above named person in any way. We do understand that participation in the Mighty Mustang Football Camp involves risk and we give our permission for above mentioned child to participate. If our child needs medical attention for an emergency, you have our permission to obtain necessary medical care and transport him for treatment to a hospital or doctor's office without any liability to 2023 WOS Mighty Mustang Football Camp or any other person associated with the camp. I certify that my child is healthy and capable of participating in the group activities. We, the undersigned parents, will bear the expense of any emergency treatment.
