Please note that if your child requires any medication to be taken whilst at camp this must be brought 'named and sealed' in a ziplock bag/medical pack to camp. It is the parent's responsibility to clearly state the dosage and administration advice of the medicine to the Camp Manager on their child's first day. *

I give photo consent for my child to be featured as part of our ASA Camp updates via the Axten Sport Academy Website/Facebook/Instagram pages. *
I am aware of the behaviour expectations set by Axten Sport Academy for all children to have an enjoyable experience. I accept that if my child does not behave in a safe and appropriate manner during their time at camp I shall be contacted and may be required to collect them from camp. *
