SAS Advocacy Reporting Form (ANONYMOUS)
This anonymous form is meant to serve as a platform for members of the SAS community to express concerns to the SAS Advocacy Chair (currently Deryl Long) without indicating their name or identity. The form does not collect names or email addresses. The advocacy chair will have sole access to the responses to this form. Since these reports are anonymous, the advocacy chair will do their best to address concerns within the community with discretion, and will not attempt to determine the identity of anyone who submits a response. There is an option to edit your response even after submission, so please feel free to update a response at any point if you see fit.

SAS Advocacy takes issues of diversity, equity, and inclusion very seriously. While this year's Advocacy Chair is not a mandatory reporter (and therefore is not obligated to "report up" to the department or the University), they may see it fit at some point to pursue restorative actions through a University department or resource if necessary, or to address concerns at a community-wide level. If this is necessary, the Advocacy chair may choose to share details of the report with department leadership, SAS leadership, or University resources. In any case, the form remains completely anonymous and the Advocacy chair will never publicly share details from a report.

Please note that this form is not an official University of Michigan or Department of Astronomy reporting mechanism. Reports can be made through the Astronomy Department by using a reporting box located by the Astronomy mailboxes on the third floor of West Hall.
