COVID-19 Rapid Test Request Form
The North American Indian Center of Boston is distributing iHealth COVID-19 Rapid Tests for Indigenous peoples, BIPOC Communities, and our neighbors in Mission Hill and Jamaica Plain.

Pick up is available on Tuesday, Wednesday, and Thursday from 11:00AM to 4:00PM.

As we continue to face the COVID-19 pandemic, at-home rapid tests are an important strategy to help
detect and prevent the spread of COVID in the community. NAICOB & the Boston Public Health Commission are
pleased to offer rapid tests to Boston residents while supplies last for your use in the comfort of your
own home.

If you have symptoms (fever, cough, headache or other symptoms) regardless of whether you have
been fully vaccinated, partially vaccinated, boosted or unvaccinated, you should test for COVID-19. If
you are unvaccinated and have been exposed to someone who tested positive for COVID-19, you
should also be tested.
