Hello Winner! If you are here to fill out this form it is because you have won a giveaway from me but have not received the prize.

Mail can take several months to be delivered, especially if international. With COVID-19 and holidays sometimes mail is even slower.

I count mail as lost after 5 months since the exact day I shipped or dropped off the mail. (Example: If I mailed the prize on 1/13/20 then the mail is counted lost on 6/13/20.) This allows for regular delays along with the possibility that the mail could be returned to me - as mail can be shipped back (if you have a tracking number this can be easily determined.)
