Please read thoroughly to be sure you are filling out the correct form.

SAME DAY ABSENCES include health/illness related absences; injury related absences; arriving late or leaving early for medical, dental, or other appointments; family emergencies; or other unexpected events. Visit https://www.mylila.org/quicklinks/report-an-absence, and fill out the ABSENT TODAY form for same day absences.

PREARRANGED EXTENDED ABSENCES are lengthier absences that need the approval of the building principal to be considered an excused absence. Families usually know about these in advance. They include: vacations; funerals; family events; religious observances; surgery; full day medical or dental appointments; full day appointments of other types; or other absences.  

This form must be completed by a parent guardian. FILL OUT ONE FORM PER CHILD. Absences will be excused at the discretion of the building principal. Failure to communicate prior to the absence may result in unexcused absences. Please note that students are responsible for making up work missed work.

We will contact you if we need additional information or if the absence is not approved for some reason. If you do not hear from us, you can assume the absence has been approved.
