Food Support Referral Form

Our food support programme is available to families with dependent children (under 18 years old) who live in the PO1, PO2, PO3 and PO4 areas only. Agencies should use this form to refer families in need of food support. Before completing this form, please obtain permission from the family to share their information.

Each family will be provided with food for up to 12 weeks. The family (or a representative from the referral agency) should COLLECT the food from the Empower Centre Portsmouth on a Thursday between 1pm and 3pm. We do not offer delivery and we cannot provide food support outside of these times.

IMPORTANT NOTE: The family (or a representative from the referral agency) will be able to collect their first weekly food parcel on the Thursday FOLLOWING their referral - or on the Thursday that the referral form is completed IF it is submitted before 10am.
