THIS FORM IS FOR APPOINTMENTS ONLY, PLEASE COMPLETE THE ILLNESS FORM IF YOUR CHILD IS GOING TO BE ABSENT.

Please use this form to let us know that you child is going to absent because of a pre-booked appointment. You can also complete this form if your child is going to be late for any other reason. If you have supporting evidence (for example a letter from the hospital), please email it to cfoffice@leoacademytrust.co.uk.
