나는 CKMC 여름학교에서 의료적 조치가 필요한 상황에서 관계자의 판단에 따라 조치할 것에 동의합니다. 또한 긴급한 상황에서 부모나 어린이의 주치의 보다 먼저 가까운 병원이나 의료기관에 연락하는 것에 동의하며 그에 따른 의료비 부담은 부모에게 있음에 동의합니다.  

I give permission to the CKMC summer program to take whatever emergency measures  are judged necessary for the care and protection of my child(ren) while under their  supervision. In case of a medical emergency, I understand that my child will be  transported to an appropriate medical facility by the local emergency unit for treatment  if the local emergency team deems it necessary. It is understood that in some medical  situations the CKMC staff will need to contact the local emergency resources before the  parents, the child’s physician, and/or other adults acting on the parents’ behalf are  notified. I understand that any expenses incurred will be borne by the child’s family.   

나는 나의 자녀가 여름학교에서 주관하는 물놀이와 야외활동, 외부 소풍에 참석하는 것에 동의합니다.  

I give permission for my child to participate in splash pool, outdoor activities and field trips during the course of the CKMC summer school.  

나는 CKMC 여름학교 활동에 참여하면서 적합하다고 간주하는 대로 비디오 이미지,  사진, 오디오 녹음 또는 기타 시각 또는 오디오 재생의 사용에 동의합니다.  

I consent to the use of any video images, photographs, audio recordings, or any other  visual or audio reproduction that may be taken of said child(ren) while participating in  activities sponsored by CKMC, to be used, distributed, or shown as CKMC sees fit.    

나는 CKMC 여름학교의 모든 프로그램에서 관계자의 직접적인 통제와 보호 아래 있음을 이해하고 참여하므로 직접 혹은 간접적으로 물질의 손실을 입었거나 부상을 당했을 때 학교나 교회 관계자 혹은 봉사자들의 고의로 인함이 아님을 인정하며 책임을 지우지 않을 것에 동의합니다.  

I understand and consent that my child will be under the direct supervision and care of  the CKMC summer school staff for its entire program. I will not hold the CKMC summer  school staff, any volunteers, or the CKMC Church responsible for any injuries or loss of  property that may happen to my child as a direct or indirect result of participating in the  CKMC summer school.
