This sent form acts as a disclaimer which governs our free online sessions.  By completing it you will access the password needed to log onto the Eginiad sessions via Zoom. Please make sure you also write it down.

1. You acknowledge the sessions may involve a risk of personal injury, including a small risk of serious injury and agree to take responsibility for your health and well-being.

2. To the best of your knowledge, you suffer from no medical or physical condition or disability that will or might increase the normal risks associated with these sessions; and / or

- your doctor has approved your participation.

- you will inform the facilitator if there is any issue they need to be aware of.  

This information will be kept securely and you can request to delete it at any point.
