LASH LIFT+TINT CHARMED POLICY AGREEMENT

These policies are put in place to protect you + your fellow clients.

A card will need to be kept on file to book any appointments. For your safety and security, it will be securely stored with your client information in the Square Booking App. *

All appointments  rescheduled or cancelled within 48-25 hours of your scheduled appointment time will be charged 50% of the price of your scheduled service. *

All appointments rescheduled or cancelled less than 24 hours before scheduled appointment time will be charged 100% of the cost of the service booked. *

Because lift+tint chemicals have to sit for a set amount of time to work properly appointments cannot be shortened for tardiness. If you are more than 10 minutes late, your appointment will be forfeited and you will be charged 100% of the cost of the service booked. *

I cannot guarantee a future appointment if you cancel or reschedule last minute. *

This agreement will remain in effect for this appointment and all future appointments conducted by my technician or any other technician conducting business at the Charmed. I understand that this agreement is binding and that I have read and fully understand all information above. I represent that I am over the age of 18 years. If below 18 years of age a parent or guardian must also sign this form. *
