Welcome! This form is a registration form for our Family Music Time SPECIAL EVENT hosted by the Azusa Conservatory of Music. We also have other classes that you can register for using our Spring 2021 registration form. Full class descriptions are listed along with prices. This form needs to be filled out for each child/person you are registering.
Music Time Info:
Ages: Baby-toddler years, This is a family participation class, parent or caregiver participate.
Time and dates: April 17th, 2021, 10:00am - 10:45am AND May 1, 2021 10:00AM - 10:45 am.

At this time, we are currently accepting ONLINE PAYMENTS for Conservatory lessons and classes. Please visit our website and navigate in the top menu to the Payments tab, or go to

Future Music Time: In the future, as we transition back into in person lessons, music time will also feature a $20 music kit that families will need to purchase (we will address this more as we get closer to going back in person).

*For all minors (under 18), legal parent or guardian must be present in person on the first zoom call lesson to confirm and approve the lessons or make a separate zoom appointment to verify and approve the lessons.
